Thank you for considering using our clinic for your vein or esthetic care. We know that payment options and price are always a concern when scheduling treatments. Our staff will be happy to assist you in determining the cost of your treatment plan and explaining your payment options. We also provide the following overview of our payment policies and plans for your convenience.
Cosmetic services are not covered by insurance plans. We do require payment in full prior to your service. Some physician services will also require a deposit in order to schedule your treatment. We accept payment by cash, check, MasterCard, Visa and Discover. If you would like to spread your payment plan out over time, we will be happy to provide information on our financing options through CareCredit.
We are pleased to offer our clients the CareCredit card, the leading patient payment plan in North America. The CareCredit card is just as easy to use as a regular credit card except that it is designed for health and beauty needs. Best of all, CareCredit offers convenient payment plans with no hidden costs. Our office can assist you in applying for this option at your visit, or you can pre-apply now in the convenience of your home. We offer both a convenient 6 and 12 month interest-free plan. You can even use your card over and over again for follow up appointments or additional treatments. Our vein procedures are typically covered by insurance. Our physician also performs a limited amount of non-cosmetic skin procedures that fall under insurance coverage. Our staff will be happy to assist you in determining your benefits and coverage.
We accept Medicare assignment. We ask that our Medicare clients bring their Medicare card to every visit. If you have secondary insurance, please inform us prior to scheduling any procedures. Some secondary insurance companies require preauthorization to receive payment. Your assistance in these matters helps us ensure that you receive your proper benefits.
Private Insurance Clients
Our nursing staff will be happy to file your claims for insurance. Many insurance plans do require preauthorization to receive payments. If preauthorization is required, our nurses will be in contact to walk you through what the process will entail. They will also keep you informed of their progress. JCMG has a policy of collection at time of service. This means that you will be required to pay your co-payments and co-insurance the day of your visit. Our front office coordinator can assist you in determining an estimate of what this fee will be prior to beginning your treatment plan. We accept cash, checks, MasterCard, Visa and Discover for these payments. If you need to spread your payment out over time, we will be happy to provide information on CareCredit's 6 and 12 month interest-free plan (see information above) or work with you to determine a payment option. This plan must be in place prior to beginning treatment. We also ask that you bring your insurance card to every visit in order to ensure your medical bills are properly paid.
Managed Care Clients
If you belong to a managed care plan that requires a referral from your primary physician, you will need to provide the referral prior to your first visit. We will be happy to assist you as needed. If you do not have your referral prior to your visit, your appointment will need to be rescheduled.
We do accept Medicaid clients in our participating plan network. A current Medicaid card must be presented at the time of your visit.
Our front office coordinator will be happy to provide a cost quote for our self-insured clients. All of the above listed payment options will be available to the self-insured client. We do require that payment be received prior to treatment unless a payment plan has been set up.
